If you have more than one pet:
- If you have more than one pet to protect, download the addendum HERE to list them.
If you want to leave money to care for your pet(s):
- Download funding instructions HERE.
Finalize and save your documents:
- Print your pet trust.

- Save your electronic (PDF) Pet Care Trust document in a safe place.
- Sign and have your trust notarized.
Notaries can be found in the following locations:
- Most banks
- FedEx Office locations(kinko's)
- UPS Stores
- On-line search for "notary locations" in Google, Yahoo, etc.
- Keep the original signed pet trust with your other legal documents.
- Attach current photos of each pet listed.
- Provide a copy to the caregiver and trustee you have chosen.
- Let your veterinarian know you have a Pet Care Trust.
Review your trust each year:
- Review your trust annually based on animal life span and the non-profit(s) you have named.
- Review and update to reflect any changes in caregiver, trustee, medical instructions, veterinarian and non-profit information. Over time your pet's care needs may change, you might want to choose someone else as caregiver, and sometimes small non-profit organizations may no longer exist.
